The Blues midfielder was highly successful with the west London club, winning four major trophies in four-and-a-half-years at Stamford Bridge.
The Spaniard made 198 appearances for Chelsea and scored 22 goals in that time.
He also rose to become the player with the second most assists (111) in Premier League history, behind Ryan Giggs only.
The midfielder now joins up with his former team-mate Thierry Henry in France with Ligue 1 side Monaco.
In a statement released on Twitter, Chelsea wrote: “Today we say goodbye and wish
@cesc4official the best of luck for the future.”
Fabregas, who is known for his defence-splitting passing ability, is leaving English football having played 501 matches for both Arsenal and Chelsea.
He became Arsenal’s youngest Premier League senior after making his debut at just 16 years old, and went on to spend eight years with the Gunners.
He then returned to his boyhood club Barcelona before returning to London to sign for Chelsea.
Fabregas made his final bow for the Blues on the weekend in Chelsea’s 2-0 win against Nottingham Forest and left the pitch in tears to a standing ovation from the crowd.
He now joins Henry’s Monaco, who are struggling in France’s top flight in 19th position.
